Professional Referral Process

At Connections Outpatient Centers, we make the process for referring patients to our virtual intensive outpatient programs (IOPs) as seamless as possible. The virtual IOPs offer flexible, comprehensive care for adults who are stepping down from higher levels of treatment or are seeking structured support while living at home. 

Getting Started

To start the process of referring a patient to one of our virtual IOPs, you can simply contact the appropriate regional team by email or phone:

The team will review the referral, confirm eligibility, and connect with you directly for next steps.  

When you reach out to us, please provide the best way for us to contact you, whether by phone or email. 

Information Needed

To ensure timely processing, please provide the following details when submitting a referral: 

  • Patient information: Demographic information and insurance provider/member ID 
  • Clinical information: Primary concern, current symptoms, treatment history, current medications (including a medication list), medical considerations, initial H&P, and initial psych evaluation 
  • Supporting documentation (if available): Recent clinical notes or discharge summary, and any applicable consent or release forms 

For internal referrals, an Acadia Facility Discharge Summary out of HMS or Best Notes can be sent to cover the necessary information. 

What Happens Next

Once your referral is received, you can expect the following: 

  • The referral team will review clinical and insurance eligibility. 
  • The admissions team will connect with you or the patient directly to complete a biopsychosocial assessment. 
  • Once the assessment is complete and it has been determined that the patient can benefit from our virtual IOP services, the team will connect with the patient to confirm participation, welcome them to the program, and schedule a start date. 
  • The patient will go through orientation and begin treatment sessions via a secure virtual platform.
